Voter Registration

The OAS is a voter registration site. Please refer to the NYS Board of Elections website for information on registering to vote. Do you require assistance with registering to vote?

Academic Accommodations

Students may submit an academic accommodation request at any point. Please note that requests may take approximately one week to process after meeting with OAS staff, and accommodations are not retroactive.

If you have documentation, such as an IEP, 504, and/or letter from your provider, please attach it in the Documentation section of this form. We recognize some students may not have access to documentation for a variety of reasons. Students may still submit a request and meet with OAS staff. Housing Accommodations

The Office of Accessibility Services (OAS) coordinates housing accommodations for students with disabilities who experience equal access barriers to the campus living environment. To request housing accommodations, please follow the steps below:

1. Complete the housing accommodation section of this form
2. Submit the treatment provider information form to the OAS
3. Meet with OAS staff

Housing Accommodation Requests should be submitted by the required submission date. If the need for the accommodation arises when an individual already resides in College housing, they should contact the OAS and complete the Request form as soon as practicably possible. Please note that requests may take 1-2 weeks after meeting with OAS staff.

Emotional Support Animals

The Office of Accessibility Services (OAS) reviews requests for Emotional Support Animals (ESA) for students with disabilities who reside on campus. To request an ESA, please follow the steps below:

1. Complete the ESA request section of this form

2. Submit the ESA treatment provider information form to the OAS

3. Meet with OAS staff

ESA requests should be submitted by the required submission date. If the need for the accommodation arises when an individual ready resides in College housing, they should contact the OAS and complete the request form as soon as practicably possible. Please note that requests may take 1-2 weeks after meeting with OAS staff.
